But after 18 cancer-free months, the disease returned with a vengeance in June 2003.

It fractured her bones and invaded her spinal canal, bathing her brain in malignancy. During the final six months, as she lay on her home hospice bed in constant pain, attached to a morphine drip around-the-clock while losing her eyesight and withering to a skeleton, the idea of ending her suffering by ending her life didn’t even enter into our conversations. I’ve been thinking a lot about those bleak days while looking into California’s End of Life Option Act, which allows terminally ill patients with a life expectancy of less than six months to end their lives by taking medications prescribed by a physician. In October, Gov.

Gavin Newsom signed a revised version of the law, extending it to January 2031 and loosening some restrictions in the 2015 version that proponents say have become barriers to dying people who wish to avail themselves of the law. The original law, which remains in effect until Jan. 1, contains numerous safeguards meant to ensure that patients are not being coerced by family members who view them as a burden or a potential financial bonanza. Under the current law, patients who want to die must make two oral requests for the medications at least 15 days apart.

They also must request the drugs in writing, and two doctors must agree the patients are legally eligible. After receiving the medications, patients must confirm their intention to die by signing a form 48 hours before ingesting them. The patients must take the drugs without assistance, either orally, rectally or through a feeding tube. And physicians can decline to prescribe the death-inducing drugs.

After the law had been in effect for a while, its proponents and practitioners concluded that some safeguards made the option unavailable to certain patients. Some patients were so sick they died during the 15 days they were required to wait between their first and second requests for the medications. Others were too weak or disoriented to sign the final attestation. The revised law reduces the 15-day waiting period to just two days and eliminates the final attestation.

It also requires health care facilities to post their aid-in-dying policies online. Doctors who decline to prescribe the drugs — whether on principle or because they don’t feel qualified — are obliged to document the patient’s request and transfer the record to any other doctor the patient designates. The most important part of the new law, advocates say, is the shortening of the waiting period. Dr.

Chandana Banerjee, an assistant clinical professor specializing in palliative medicine at City of Hope National Medical Center in Duarte, California, says she’s seen many patients who were afraid to broach the subject of their own death until they were very close to the end. €œBy the time they made that first request and then had to wait for those 15 days, they either became completely nondecisional or went into a coma or passed away,” Banerjee says. Amanda Villegas, 30, of Ontario, California, became an outspoken advocate for updating the law after watching her husband, Chris, die an agonizing death from metastatic bladder cancer in 2019. When the couple asked about the possibility of a physician-assisted death, Villegas says, staff at the Seventh-day Adventist hospital where Chris was being treated told them, inaccurately, that it was illegal.

When he finally made a request for death-inducing drugs, it was too late. He died before the 15-day waiting period was up. The new law “will open doors for people who might … experience the same roadblocks,” Villegas says. €œWhen you are dying, the last thing you need is to go through bureaucratic barriers to access peace.” Medical aid in dying straddles the same political fault line as abortion and has long been opposed by many religious institutions and anti-abortion groups.

It has also encountered resistance from some disability rights organizations that claim it belittles the lives of those who are physically dependent on others. €œWe object to the whole idea of a state providing a vehicle for people to kill themselves,” says Alexandra Snyder, CEO of the Life Legal Defense Foundation, an anti-abortion nonprofit law firm. The 15-day waiting period, she says, provided an important cooling-off period for patients to reflect on a decision that is irreversible. €œNow, any safeguards that were in the law are gone.” Proponents of the law say they haven’t seen evidence that a patient’s decision to take the life-ending medications has ever been anything other than voluntary.

Though neither version of the law requires a medical professional to be present when a patient takes the drugs, medical standards encourage professional participation in the dying process, says Dr. Lonny Shavelson, chair of the American Clinicians Academy on Medical Aid in Dying. The drugs should be kept at the pharmacy until the patient is ready to ingest them, he says — though that doesn’t always happen. From the time the aid-in-dying law took effect in June 2016 through Dec.

31, 2020, just under two-thirds of the 2,858 people who received prescriptions actually took the medications and died, according to the most recent data from the California Department of Public Health. The rest died before they could take the drugs or found other ways to manage their pain and emotional distress. Most major commercial health plans — including Kaiser Permanente, Anthem Blue Cross, Blue Shield of California and Health Net — cover aid-in-dying drugs and the related doctor visits, as does Medi-Cal, the government-run health insurance program for people with low incomes. However, more than 60% of those who take the drugs are on Medicare, which does not cover them.

Effective life-ending drug combinations are available for as little as $400. If you are contemplating aid in dying, here are some resources to learn more about it. Talk to your doctor sooner rather than later if you are considering medically assisted death, in case you’ll need to seek a different physician’s help. If your doctor agrees to help, the law requires that he or she discuss other end-of-life options with you, including hospice and palliative care.

Whatever choice a patient makes, being more open about our mortality is important, says state Sen. Susan Eggman (D-Stockton), the author of the revised aid-in-dying law. €œWe should all have more conversations about life and death and what we want and don’t want and what is a peaceful death,” she says. €œWe are all going to die.” Leslie and I had plenty of those conversations, until the cancer ultimately robbed her of her mind.

In her final months, she would often sink into a semiconscious state, far out of our reach for days on end. Then, just when we thought she wasn’t coming back, she would suddenly open her eyes and ask for the children. She’d get into her wheelchair and join us at the dinner table. Those periods, which we called “awakenings,” were a source of great comfort to everyone who loved Leslie — but especially to the kids, who were relieved and overjoyed to have their mommy back.

Specifically, the word “assistant.” Patients have asked if she’s heading to medical school or in the middle of it. The term confounded even her family, she said.

It took years for her parents to understand she did more than take blood pressure and perform similar basic tasks. €œThere is an assumption that there has to be some sort of direct, hands-on oversight for us to do our work, and that’s not been accurate for decades,” said Clayton, who practices at a clinic in Golden Valley, Minnesota. €œWe don’t assist. We provide care as part of a team.” Seeking greater respect for their profession, physician assistants are pushing to rebrand themselves as “physician associates.” Their national group formally replaced “assistant” with “associate” in its name in May, transforming into the American Academy of Physician Associates.

The group wants state legislatures and regulatory bodies to legally enshrine the name change in statutes and rules. The association estimates the entire cost of revising the profession’s title will reach nearly $22 million. Rechristening the P.A. Name has spiked the blood pressures of physicians.

They complain that some patients will wrongly assume a “physician associate” is a junior doctor, much as an attorney who has not yet made partner is an associate. The head of the American Medical Association warned that the change “will undoubtedly confuse patients and is clearly an attempt to advance their pursuit toward independent practice.” The American Osteopathic Association, another group that represents doctors, accused the P.A.s and other nonphysician clinicians of trying “to obfuscate their credentials through title misappropriation.” “There is an assumption that there has to be some sort of direct, hands-on oversight for us to do our work, and that’s not been accurate for decades,” says Clayton.(Liam James Doyle for KHN) In medicine, seemingly innocuous title changes are inflamed by the unending turf wars between various levels of practitioners who jealously guard their professional prerogatives and the kind of care they are authorized to perform. Just this year, the National Conference of State Legislatures catalogued 280 bills introduced in statehouses to modify so-called scope-of-practice laws that set the practice boundaries of nurses, physician assistants, pharmacists, paramedics, dental hygienists, optometrists and addiction counselors. Lawmakers let North Carolina dental hygienists administer local anesthetics.

Permitted Wyoming optometrists — who, unlike ophthalmologists, do not attend medical school — to use lasers and perform surgeries in certain circumstances. And authorized Arkansas certified nurse practitioners to practice independently. The physicians’ lobby aggressively fights these kinds of proposals in state legislatures, accusing other disciplines of trying to incrementally horn in on things doctors claim only they are competent to do. Physician assistants, as they are still legally called, have been steadily granted greater autonomy over the years since 1967, when the Duke University School of Medicine graduated four former Navy medics as the nation’s first class of P.A.s.

Today they can perform many of the routine tasks of doctors, such as examining patients, prescribing most kinds of medications and ordering tests. In most states, all that usually happens without the need for a physician signoff or having a physician in the same room or even in the same building. The profession is pressing for more. It wants to abolish state mandates that P.A.s must be formally supervised by physicians or have written agreements with a doctor spelling out the P.A.’s role.

Generally, a P.A. Master’s degree takes 27 months to earn and includes about 2,000 hours of clinical work. By comparison, family physicians usually attend four years of medical school and then do three-year residencies during which they clock about 10,000 hours. (Specialists spend even more time in residencies.) Nearly 150,000 P.A.s were practicing in 2020 in the U.S.

Their median annual pay that year was $115,390, slightly above the $111,680 median pay for nurse practitioners, who perform jobs similar to P.A.s. The median annual pay for a family physician was $207,380. P.A.s aren’t alone in losing patience with their titles. In August, the American Association of Nurse Anesthetists renamed itself the American Association of Nurse Anesthesiology — its third name since it was founded in 1931.

President Dina Velocci said the term “anesthetist” baffles the public and is hard to pronounce, even when she helps people sound out each syllable. (It’s uh-NES-thuh-tist in the U.S. And indicates a registered nurse, usually with a bachelor’s degree in nursing, who has then received several more years of education and training in anesthesia.) The association’s new name is justified since “we’re doing the lion’s share of all the anesthetics in this country,” Velocci said. €œI’m definitely not trying to say I’m a physician.

I’m clearly using ‘nurse’ in front of it.” Physicians’ groups have condemned the change, though the legal title for the profession remains certified registered nurse anesthetist, or CRNA. Likewise, the P.A.s say there’s no ulterior motive in altering their name. €œChanging the title is really just to address that misperception that we only assist,” said Jennifer Orozco, president of the P.A. Association and an administrator at Rush University Medical Center in Chicago.

€œIt won’t change what we do.” They say “assistant” confuses not just patients but also state lawmakers and those who hire medical professionals. When Clayton recently testified before Minnesota legislators about a scope-of-practice bill, she said, lawmakers “just couldn’t get their heads around” the concept of “an assistant who doesn’t have a direct supervisor.” The message she said they gave her. €œYou guys really need to do something about your title.” The P.A. Association’s consultants developed more than 100 alternatives, including “medical care practitioner” and the widely derided neologism “praxician.” “Physician associate” won out thanks to several advantages.

It allowed P.A.s to continue to introduce themselves with the same initials, and it had been flirted with as an alternative throughout the profession’s history to distinguish the most highly trained P.A.s from those with less training. The association even briefly used “associate” in its name for two years in the 1970s, and Yale School of Medicine has offered a physician associate degree since 1971. But a name change alone won’t resolve other disadvantages P.A.s face. In some states, doctors are required to meet regularly with P.A.s, periodically visit them in person if they work at a different location and review sample patient charts on a recurring basis.

States generally mandate less oversight for nurse practitioners, making them more appealing to some employers. €œWe’ve heard from our P.A. Colleagues that they’re getting passed over for jobs by nurse practitioners,” said April Stouder, associate director of the Duke Physician Assistant Program. Many physicians offer concerns about patient safety if P.A.s drift too far from their oversight.

Dr. Colene Arnold, a gynecologist in Newington, New Hampshire, started her medical career as a P.A., practicing with little supervision. In retrospect, she said, “I didn’t recognize the severity of what I was seeing, and that’s scary.” Dr. Kevin Klauer, CEO of the osteopathic association, said misdiagnoses by a solo P.A.

Are more likely than when a physician is involved. €œIf you go to Jiffy Lube and you want an oil change and a tire rotation, that’s what they’re going to do,” he said. €œMedicine is not like that.” Orozco, the P.A. Association president, said such anxieties are overblown.

€œThey will always collaborate with physicians and really want to keep working in that team-based environment,” she said. Doctors should welcome P.A.s to help fill physician shortages in primary care, behavioral health and telemedicine and free up doctors to focus on complex cases, she added. €œI can have a jet engine mechanic change the tires on my car,” she said, “but do I need that every single time?. € Jordan Rau.

Home health aide Sharon Coleman looks forward to having coverage that will cover a hospital stay. Incoming medical student Danielle Gaddis no longer worries a trip to the doctor will leave her in debt. All three are among the roughly 490,000 people newly eligible for Medicaid after Oklahoma and Missouri voters in 2020 approved expanding the federal-state public health insurance program for people with low incomes.

In both states, people who earn up to 138% of the federal poverty level — about $18,000 per year for an individual — can now get the free coverage even if they don’t have disabilities. But the experience of the newly eligible in those two states — an estimated 215,000 people in Oklahoma and 275,000 in Missouri — has differed dramatically. Oklahoma has enrolled over 210,000 people, while Missouri has enrolled fewer than 20,000. The difference comes down to the approaches taken by the two states, both of which are Republican-led and resisted expanding Medicaid for years.

Once Oklahoma voters approved the expansion, it was quickly embraced. The legislature appropriated $164 million in the state budget to fund it. Applications opened this June, one month before the program began, and within a month, 113,000 people had been approved. In August, Oklahoma Secretary of Health and Mental Health Kevin Corbett told state lawmakers about one of those enrollees.

A 62-year-old woman able to schedule appointments with a doctor and dentist for the first time in 20 years. €œTruly life-changing,” Corbett said. €œWe feel very good about what we’re being able to do.” Other states that expanded Medicaid in recent years saw enrollment swell in the first month. Louisiana’s combined Medicaid and Children’s Health Insurance Program rolls grew by over 255,000.

Virginia’s increased by almost 184,000. Idaho added about 45,500 enrollees, roughly half of the anticipated number of newly eligible people. And Montana added over 23,000, 51% of its expected total. Missouri, on the other hand, has enrolled only about 7% of newly eligible people in Medicaid.

€œYou can expand Medicaid on the books, but there are a lot of ways that you can throw up barriers to keep people from getting enrolled,” said Sidney Watson, director of the Center for Health Law Studies at Saint Louis University. Expansion has been a slog in Missouri. The legislature refused to fund the voter-approved program, prompting Republican Gov. Mike Parson to announce in May that the state would “withdraw” its expansion plan.

Then, in August, a judge ordered the state to start accepting applications, which it did. But Missouri could not begin processing them until Oct. 1. That whiplash means many newly qualified Missourians are likely unaware they could be covered by Medicaid, according to University of Missouri-Kansas City law professor Ann Marie Marciarille.

Marciarille said that the state has a responsibility to get the word out about the program and that Missouri has done little more than what was legally required by the August court order. The critique has been echoed by others. Sunni Johnson, a certified application counselor at Affinia Healthcare in St. Louis, Missouri, files paper copies of the Medicaid applications she completed online.

The absence of robust advertising by the state about Medicaid expansion means much of the outreach effort falls to clinics like Affinia. Johnson says she and her colleagues at the clinic complete about 20 Medicaid applications per day. (Bram Sable-Smith/KHN) Heather Dolce, a spokesperson for the Missouri Department of Social Services, which oversees Medicaid, said the department had publicized the Medicaid expansion by updating its website, emailing participants in its family support programs and posting on social media. The department’s Facebook and Twitter accounts show a handful of posts about the expansion, including two tweets posted the day after the publication of a KCUR article that noted the state’s outreach efforts had been slow.

Oklahoma’s efforts, meanwhile, included outreach events, TV interviews, plus a video and social media campaign. In Missouri, without robust advertising by the state, much of the outreach has fallen to clinics like Affinia Healthcare in St. Louis. James Dickerson saw a flyer about the Medicaid expansion on the front door of an Affinia clinic when he went to see a doctor about an ear .

The 59-year-old, who works various jobs through a temp agency, was eager to sign up. He had a good experience with Medicaid in 2014 when he was covered while getting spinal surgery for a workplace injury. At Affinia, Dickerson met with a certified application counselor, Sunni Johnson, who within about five minutes got all the information she needed to send in his application. Most clinics like this have specialists who can help patients enroll in health insurance and other assistance programs.

By federal law, Missouri is required to determine whether an applicant is eligible for the program within 45 days. But Michelle Davis Reed, the lead eligibility and enrollment coordinator at Northwest Health Services, based in St. Joseph, Missouri, said in November that some applications she had filed in August were still not processed. Dolce said the state had 32,000 Medicaid applications pending as of Nov.

17. She did not respond directly to a question about the number of staffers processing the applications but said overtime was being used. In Oklahoma, 144,000 of the 210,000 Medicaid expansion enrollees previously lacked insurance. The remainder were enrolled when the state evaluated whether people who had applied to other benefit programs were also now eligible for Medicaid.

One Missouri program that could be a candidate for such reprocessing is Gateway to Better Health, a temporary insurance program that Sharon Coleman uses in St. Louis. Gateway covers about 16,000 residents of the city of St. Louis and St.

Louis County who earn up to 100% of the federal poverty level. Louis Regional Health Commission, which administers Gateway, said she believes that at least 90% of its participants would qualify for Medicaid under expansion but that bureaucratic hurdles made encouraging patients to sign up for Medicaid directly simpler than the process necessary to move them into a new system. Gateway sent a text message to Coleman and other recipients urging them to apply. After getting the alert, Coleman, 60, who provides in-home care to older adults, went to Affinia specifically to enroll in Medicaid. She has high blood pressure, but her health is otherwise good — she said she hasn’t been to a hospital since her son was born 40 years ago.

Coleman was relieved to know that if she had to go now, her bills would be covered by Medicaid. Gateway covers only primary, specialty and urgent care. €œI can go to the emergency room now and not worry about them sending me bills I can’t pay,” Coleman said after visiting with Johnson, the enrollment specialist. In Oklahoma, 26-year-old Danielle Gaddis had been uninsured for two years.

She was reluctant to see a doctor during that time, fearing the medical bills. So she was thankful to be enrolled in Oklahoma’s Medicaid program when she got sick recently. Like Dickerson and Coleman, Gaddis applied for Medicaid with the help of a specialist at a health clinic, Mary Mahoney Memorial Health Center in Oklahoma City. €œBecause of buy antibiotics, a cold could be the end of the world, so you freak out,” said Gaddis, who will start medical school in August after deferring for a year.

During Supreme Court arguments over the Mississippi abortion law, some liberal justices questioned whether anything had changed in the country that should cause the court to reverse the long-standing precedent. One historic event, however, did play a key role in the case. The September 2020 death of Justice Ruth Bader Ginsburg, who was a staunch defender of the right to abortion, and her subsequent replacement by Justice Amy Coney Barrett, who has written about her concerns with the Roe opinion. In fact, the court delayed for months a decision on whether to take the Mississippi case, doing so only after Ginsburg’s death.When Mississippi initially petitioned for relief from the Supreme Court — after its abortion law had been overturned by lower federal courts — the state said it sought only the ability to reduce the time frame for permissible abortions from the current standard of roughly 24 weeks to 15 weeks.

But once the case was accepted, Mississippi changed its arguments and asked the court to overturn the full effect of the Roe decision.The attorney representing Mississippi’s abortion clinic and the U.S. Solicitor general argued to the court that if Mississippi is allowed to ban most abortions after 15 weeks, there will be nothing to stop other states from establishing even tighter time frames.The justices just last month rushed to hear a case in which Texas is seeking to ban abortions after six weeks. Those arguments had more to do with the legal process for appealing the law and the unusual enforcement of that law, so a decision in the Texas case won’t necessarily indicate much about the justices’ thinking on the Mississippi law or abortion rights in general.Nonetheless, the Texas situation is affecting Mississippi’s sole abortion clinic. The court allowed the Texas law to go forward during the legal review, effectively shutting down most abortion services in the state.

Many Texas residents seeking abortions must now travel to get the procedure, and many are heading to Mississippi’s clinic.Congress has a long to-do list before heading home at the end of the year. It has not yet resolved spending bills or extended the debt ceiling. Those priorities could force Senate Majority Leader Chuck Schumer to push back final votes until 2022 on the Build Back Better legislation, which would fund President Joe Biden’s social policy agenda.The Senate parliamentarian is going through that massive bill to make sure each provision is primarily about federal spending. But few are qualified to help more critical students, like those with mental illness or who zithromax generic price have experienced trauma. Woodward Public Schools reported 82 homeless students during the 2017-18 school year – more than twice the state average. Nearly two-thirds of students at Woodward and Elk City Public Schools qualified for free and reduced lunches, compared to the state’s average of 50%.

Elk City, Woodward and Weatherford districts hoped to bring more licensed professional counselors into schools by hiring new staff and using Project AWARE funds to pay for training for current school counselors. The state requires school counselors to have a master’s degree in a related field or two years of experience, and pass the state’s general education, professional teaching and school counseling exams. "When you look at what's happening here (in Washakie County, it) is just the tip of the iceberg," said Franklin. "There's other services throughout the state that are just one bad year away from closure."'A matter of life and death'One of those is Fremont County -- home to the Wind River Indian Reservation. Fremont is roughly the size of the state of Vermont.

An economic downturn and budget cuts prompted the county to privatize its ambulance service in 2016. But the private company, American Medical Response, says it can't afford to keep going after losing $1.5 million in revenue last year. AMR announced it won't renew its contract when it runs out on June 30.

No others have bid. "We just couldn't renew that current contract because it was set up for a financial failure," said Matt Strauss, Regional Director for AMR parent company, Global Medical Response. One of the problems, according to Fremont County Commissioner Larry Allen, is the so-called payer mix.

Many of the county's residents rely on Medicare, Medicaid and Indian Health Services, which reimburse ambulance providers at a lower rate. And without state or federal designation of EMS as an essential service, Allen said "there's no source of revenue to operate an ambulance.""Because of the distance and the ruralness of this county, we just don't have people standing in line wanting to provide ambulance service," Allen said. The Wind River Indian Reservation stretches across more than 2 million acres and is shared by two Native American tribes, the Eastern Shoshone and the Northern Arapaho.

It has three tiny clinics but no ambulance services and relies on Fremont County for EMS. "Right now the response time is pretty slow and it's going to be nonexistent," said Northern Arapaho tribal member Juan Willow. His grandfather struggled with health problems and Willow said there were many times when the family couldn't wait for an ambulance and had to find other ways of getting to the hospital.

"Not everyone here has a car," he said.It's a concern shared by Jordan Dresser, the chairman of the Northern Arapaho Tribe."I think if we didn't have access to ambulances, death rates would be higher," said Dresser, adding that many tribal members don't have working vehicles and therefore can't take themselves to the hospital or clinics. "It's a matter of life and death for us.".

But few are qualified to help more critical students, like those with mental illness or who have experienced zithromax prices walmart trauma. Woodward Public Schools reported 82 homeless students during the 2017-18 school year – more than twice the state average. Nearly two-thirds of students at Woodward and Elk City Public Schools qualified for free and reduced lunches, compared to the state’s average of 50%. Elk City, Woodward and Weatherford districts hoped to bring more licensed professional counselors into schools by hiring new staff and using Project AWARE funds to pay for training for current school counselors. It has three tiny clinics but no ambulance services and relies on Fremont County for EMS. "Right now the response time is pretty slow and it's going to be nonexistent," said Northern Arapaho tribal member Juan Willow. His grandfather struggled with health problems and Willow said there were many times when the family couldn't wait for an ambulance and had to find other ways of getting to the hospital. "Not everyone here has a car," he said.It's a concern shared by Jordan Dresser, the chairman of the Northern Arapaho Tribe."I think if we didn't have access to ambulances, death rates would be higher," said Dresser, adding that many tribal members don't have working vehicles and therefore can't take themselves to the hospital or clinics.

The excess scar tissue that forms as a result of this type of hair loss may also explain the higher risk for uterine fibroids, which are characterized by fibrous growths in the lining of the womb.
